SENDING NEW PRESCRIPTIONS
Customers who have previously ordered can send new prescriptions
by mail or you can have your doctor fax the prescriptions
directly to us. Please note: you do not need to send us
any forms along with it.
Please make sure to have at least two of the following attached
to your prescriptions:
a) First and Last Name
b) Customer ID#(please check invoice)
c) Birth Date
This information is to ensure that an order is being sent
out to the right customer.
